Please make a separate appointment for each year in which you need a return prepared. Also please make a separate appointment for each individual who is having a separate return prepared. Preparing a return for you and a separate return for a parent or child requires a separate appointment for each one.

Please pick up a Taxpayer Packet at TDYCC reception before your appointment. This packet contains the Intake Interview Form, which you will need to fill out as completely as possible before your appointment. It also contains a detailed list of documents you need to bring to your appointment.

Key documents you will need to bring to your appointment:

• Government-issued photo ID for you and your spouse (if married)

• Front of your Social Security card (and that of your spouse, if married)

• Front of the Social Security card OR ITIN number for each of your dependents named on your return

• Intake/Interview & Quality Review Sheet which is included in your Taxpayer Packet
