Please make a separate appointment for each year in which you need a return prepared. Also please make a separate appointment for each individual who is having a separate return prepared. Preparing a return for you an a separate return for a parent or child requires a separate appointment for each one.

Key documents you will need to bring to your appointment:

• Government-issued photo ID for you and your spouse (if married)

• Front of your Social Security card (and that of your spouse, if married)

• Front of the Social Security card OR ITIN number for each of your dependents named on your return
